On 2 November 1968, Specialist Four Ralph R. Carpenter, Jr. was serving with D Company, 3rd Battalion, 12th Infantry Regiment, 4th Infantry Division, in Kontum Province, South Vietnam.

On that day, SP4 Carpenter was killed in action when he sustained wounds from small arms fire. His body was recovered.
